Release of Liability Form

All participants in OCVA programs are required to review and acknowledge this waiver before their first session. Please read each section carefully.

Before You Begin

All participants under 18 must have a parent or legal guardian sign this waiver
This waiver must be completed before the participant's first session
One waiver covers all OCVA programs for the current season
A new waiver is required at the start of each new season or calendar year

Assumption of Risk

I understand that participation in volleyball activities, including but not limited to training sessions, camps, clinics, leagues, and tournaments, involves inherent risks of physical injury. These risks include but are not limited to sprains, strains, fractures, concussions, and other injuries that may result from physical contact, falls, or the use of equipment. I voluntarily assume all risks associated with participation.

Release of Liability

In consideration of being permitted to participate in OCVA programs, I hereby release, waive, discharge, and covenant not to sue the Orange County Volleyball Association (OCVA), its officers, directors, coaches, volunteers, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during participation.

Medical Authorization

In the event of an emergency, I authorize OCVA staff and coaches to seek and obtain emergency medical treatment for the participant. I understand that I am responsible for all medical costs and expenses incurred. I confirm that the participant is physically fit and has no medical conditions that would prevent safe participation, or that any such conditions have been disclosed in writing to OCVA.

Photo & Video Release

I grant OCVA permission to use photographs, video recordings, and other media captured during programs for promotional, educational, and marketing purposes, including use on the OCVA website, social media, and printed materials. If you wish to opt out of media use, please notify our team in writing before the program begins.

Code of Conduct

All participants and their families agree to maintain respectful and sportsmanlike behavior at all times. OCVA reserves the right to dismiss any participant whose conduct is deemed disruptive, unsafe, or contrary to the values of the organization, without refund.

Acknowledgment

By registering for any OCVA program, you acknowledge that you have read, understood, and agree to the terms outlined in this waiver. For participants under 18 years of age, a parent or legal guardian must provide consent on their behalf. This waiver is binding for the duration of the participant's enrollment in OCVA programs for the current season.
